The Federal Occupational Health (FOH) office in Downtown Tampa is providing
all required immunizations to Federal (FEMA/NDMS) workers at
no charge. The FHO office is located at:

To get your immunization shots, you must bring your FEMA ID card
and immunizations records with you! Please contact Gloria Halber for
an appointment.
NOTE: YOU MUST SHOW YOUR FEMA ID TO GET INTO THE BUILDING AND TO GET YOUR IMMUNIZATIONS.
In order to deploy you must have the following shots:
Hepatitis A
Hepatitis B (HBV)
Influenza (annual)
Measles, Mumps, Rubella (MMR)
Poliovirus (IPV)
Tetanus-diphtheria (Td)
Varicella
Tuberculin Skin Test (annual)
Pneumococcal Polysassharide Vaccine (Not a requirement)
