The Federal Occupational Health (FOH) office in Downtown Tampa is providing all required immunizations to Federal (FEMA/NDMS) workers at no charge. The FHO office is located at:

To get your immunization shots, you must bring your FEMA ID card and immunizations records with you! Please contact Gloria Halber for an appointment.

NOTE: YOU MUST SHOW YOUR FEMA ID TO GET INTO THE BUILDING AND TO GET YOUR IMMUNIZATIONS.

In order to deploy you must have the following shots:
  • Hepatitis A
  • Hepatitis B (HBV)
  • Influenza (annual)
  • Measles, Mumps, Rubella (MMR)
  • Poliovirus (IPV)
  • Tetanus-diphtheria (Td)
  • Varicella
  • Tuberculin Skin Test (annual)
  • Pneumococcal Polysassharide Vaccine (Not a requirement)
  • Summary of Recommendations for Adult Immunizations.
